Congregate and Home Delivered Nutrition Services

The State of Delaware Department of Health and Social Services is seeking professional services for Congregate Nutrition and Home Delivered Nutrition Services. The services include providing meals and related nutrition services to older individuals in congregate settings and homebound older adults. The proposal must include a work plan, corporate qualifications, and cost analysis. The contract is expected to be in effect until at least September 30, 2027. Key responsibilities of the selected vendor will include providing nutrition education, nutrition counseling, and other nutrition services as needed. The vendor will also be required to comply with the State of Delaware's terms and conditions, including data usage and security requirements. Some of the key points to consider are: • The vendor must provide proof of Delaware license(s) and/or certification(s) necessary to perform services. • The vendor must provide responses to the Request for Proposal (RFP) scope of work and clearly identify capabilities. • The vendor must complete all appropriate attachments and forms as identified within the RFP. • The vendor must provide proof of insurance and amount of insurance prior to the start of the contract period.
