Guardianship and Conservatorship Services

The City of Norfolk's Department of Human Services is seeking qualified vendors to provide combined Guardianship and Conservatorship services for residents 60 years of age or older or any person 18 years of age or older who is incapacitated and has been adjudicated by the court as needing assistance with both personal and financial affairs. The contractor will be responsible for providing both services together under a single contract and performing all responsibilities listed. The City anticipates multiple contract awards to qualified offerors, each for a five-year term. Some key responsibilities include maintaining sufficient contact with the incapacitated adult, visiting the adult at least three times per year, making decisions regarding health, safety, residence, and personal care in the adult's best interest, and seeking prior court authorization for major decisions. • Provide combined Guardianship and Conservatorship services • Maintain sufficient contact with the incapacitated adult • Visit the adult at least three times per year • Make decisions regarding health, safety, residence, and personal care in the adult's best interest • Seek prior court authorization for major decisions • File annual reports on the adult's condition with DHS and the court • Ensure communication rights and do not unreasonably restrict the adult's ability to communicate or interact with people with whom they have an established relationship
