Description
The USDA is seeking a small‑business vendor to provide on‑site medical services at the Jacobs Creek Job Corps Center in Bristol, TN. The contract is a firm‑fixed‑price RFQ for a one‑year base period with up to four one‑year options and a possible six‑month extension, totaling up to five years (plus extension). The award will cover 400 hours of clinical work per year, which may be performed by a physician, NP, PA, or a combination thereof. Vendors must meet strict credentialing, insurance, and licensing requirements and submit complete pricing for each personnel line item.
• 400 hours of qualified medical care annually (examinations, treatment, immunizations, chronic‑care management, etc.)
• Flexible staffing: single MD/NP/PA or split MD + NP/PA, each line item priced separately
